In 2022, the Washington University NF Center established one of the first comprehensive Adolescent and Young Adult (AYA) Clinical Program to seamlessly transition our children from pediatric to adult NF care.

Our Approach

In order to transition individuals from pediatric to adult NF care, we have created a series of steps to ensure success.


  1. At 12 years of age, all children are invited to participate in our Totally TEEN social and life skills program.
  2. At 14 years of age, all children complete an annual assessment to determine their level of independence.
  3. By 17 years of age, adult and pediatric NF providers meet bimonthly to assign an appropriate adult provider and make transition plans specific for that patient.
  4. Once all members of the healthcare team (including the patient and their parents/caregivers) are confident in the individual’s ability to transition to the adult healthcare system, either direct transition or overlapping appointments between the pediatric NF provider and the designated adult NF provider are scheduled.
